It's a slow movie but I liked it. It's sad to eventually find out that she committed suicide. I haven't seen the Japanese version so I didn't know what to expect.

*SPOILER WARNING!*

How do you know it was suicide?
There has been some fan speculation that:

1. She might have also died; hence, how she was able to be with him in the end.
2. They might have both survived, since there was an ambulance siren in the background. She was, after all, able to see.

It is a good possibility that she committed suicide since she's made it known that she wanted to die, throughout the film. The likelihood of him surviving was slim, especially after he closed his eyes for, in some people's POVs, the last time and the ambulance probably did not make it on time. The ending scene was just a sort of "happy ending" in the afterlife or something. As I type all these possibilities out, I really do see the logic in your assumption; however, I'm afraid that I do have my doubts over the suicide notion, as I do about the first two I had mentioned previously.

btw, when I said, in the very first "speculation", also died-- I meant, naturally, as in, she never had the operation or the operation had failed.

I'm pretty sure she didn't go to surgery or didn't make it through surgery because there were white cloths all over the furniture. Hence, no one living at that home because everyone died.
